Friendsday Wednesday Parent Permission Form
Each month, a group of students journeys to D’Youville Health and Wellness Center
to bring companionship and friendly faces to the residents. Students can sign up by emailing Mrs. Deignan at [email protected]. Participants must have both initial doses of the COVID19 vaccine and their vaccination card must be sent the week prior to a visit. Parents also need to fill out the digital permission form before a student’s initial visit. Once a form has been filled out and vaccination cards sent, the only requirement is to email Mrs. Deignan to request a spot each month. Space is limited to 6 students each month.
