Massachusetts School Immunization Requirements

Maintaining a safe and healthy learning environment for students, faculty and staff is a top priority at the Academy of Notre Dame.
 
Massachusetts state law requires that all students have up-to-date immunizations in order to attend school in person.  Legally, children are not allowed to stay in school until they have received all required immunizations for their age.
 
This law applies to both international and domestic students.  
 

GRADES 7-12

Students in grades 7-12 are required to have the following immunizations:
  • A physical within the last year
  • Tdap = 1 dose
  • Polio = 4 doses
  • Hepatitis B = 3 doses
  • MMR = 2 doses
  • Varicella =2 doses
 
Meningoccal requirements:
  • Grades 7-10 = 1 dose
  • Grades 11-12 = 2 doses
 

GRADES Pre-K 3 - 6

Students in grades Pre-K 3 and Pre-K 4 are required to have the following immunizations:
  • Hib = 1-4 doses
  • DTaP = 4 doses
  • Polio = 3 doses
  • Hepatitis B = 3 doses
  • MMR = 1 dose
  • Varicella = 1 dose
 
Students in grades Kindergarten -6 are required to have the following immunizations:
  • DTaP/Tdap = 5 doses
  • Polio = 4 doses
  • Hepatitisi B = 3 doses
  • MMR = 2 doses
  • Varicella = 2 doses
